Unfortunately, the steady increase during the 1970s in the proportion of women who begin prenatal care in the first trimester og pregnancy has ceased in the 1980s. And in 1985, for the sixth consecutive year, no progres was made in reducing the percentage of infants born to women who begin care only in the third trimester or not at all. These disturbing trends present important challenges to public policy and to the health care system for several reasons. This country's limited progress in extending prenatal care is only part of a larger problem of poor access to health services for low-income and oriented medical care system, the United States has failed to find adequate ways to finance health care for the poor. In 1988, socioeconomic status remains a major determinant of both health status and use of medical services. Faced with evidence of prenatal care's value and cost-effectiveness, and with data revealing poor and declining use of this key service, in the summer of 1986 the Institute of Medicine convened an interdisciplinary committee, the Committee to Study Outreach for Prenatal Care, to study ways of drawing more women into prenatal care early in pregnancy and of sustaining their participation until delivery.
